Who is the Trafford Co-ordination Centre for?
The TCC is for any resident in the area who is registered with a Trafford GP. If you think you or your relative would benefit from this service, you can ring the TCC on 0161 976 2555 or talk to your GP about it.
When should I contact the Trafford Co-ordination Centre and not my GP?
If you have been discharged from hospital through the TCC or have been referred by your GP for the care co-ordination service and have concerns about your care, then you should contact the TCC. If it is a medical emergency, you should contact your GP or NHS 111, if it is out of normal surgery hours.
I want to talk to someone about the service I have received from the Trafford
Co-ordination Centre, what should I do?

Am I eligible for hospital transport?
If you are registered with a Trafford GP and need to attend a hospital appointment and have a medical condition or mobility issues that mean you can’t make your own way there, then you might be eligible for non-emergency patient transport. You can talk to the person booking your appointment about this.
