Trafford Co-ordination Centre
The Trafford Co-ordination Centre (TCC) is a new approach to co-ordinating care. It has been designed to deliver an efficient and effective way for Trafford residents, their families and carers to access health and social care services. The TCC works in partnership with health and social care organisations across Trafford to ensure people can get the right care, at the right time, in the right place.
The TCC delivers a streamlined referral process, that aims to get people to their appointment at the hospital with all relevant information in place.
The Trafford Co-ordination Centre is a central point of contact which co-ordinates health and social care for people with multiple needs.
The whole ethos of the service is to equip people to manage their healthcare, keep them comfortable at home and offer non-medical options where appropriate.
Who we are
The Trafford Co-ordination Centre has a dedicated team of administrators and clinicians working to provide help and support 5 days a week.
The administrators ensure all referrals received into the Centre contain the correct information before booking the most suitable appointment for the patient. They also book hospital transport for eligible patients.
The clinical team come from a variety of nursing backgrounds and are supported by a GP. They provide a telephone support service to patients enrolled onto the Care Co-ordination service. Their expertise allows them to identify potential problems and intervene at an early stage, to help prevent unnecessary admissions or re-admissions to hospital. Our Nurses are passionate about ensuring patients stay safe and supported at home and maintain a good quality of life.
